If you have been injured at work and have filed an accident report, we will bill your employer’s Work Comp carrier for you. You must bring with you an authorization for treatment from your employer which should contain their Work Comp carrier, billing address, date of injury, and claim number.
There are many patients who carry disability or accident insurance. We will gladly complete your form promptly. If it needs to be mailed, please provide a stamped and addressed envelope. The first form will be done as a courtesy, and additional forms will be completed for a fee of $10.00 each.
We stay abreast of most insurances and their benefits but it is impossible to know them all.
We participate with Medicare, Blue Cross, Blue Cross PPO, Tricare Standard, Aetna, Priority Health and Cofinity. We accept Blue Care Network, Tricare Prime and Medicaid when referred by your primary care physician with appropriate authorization.
Our initial office visit consultation fee will range depending on the complexity of your history and current problem. X-rays and injections are additional.
We expect payment of office exam charges on the date of visit, except as we are governed by our participation in the listed plans. Be prepared to pay your co-pay at the time of your office visit. If you are unable to do this, you must speak to the receptionist prior to the exam. For your convenience, we accept VISA and Mastercard. Checks should be made to ARMC.

Under-age children must be accompanied by a parent or guardian. The parent or guardian present with a minor child at the time of service will be held responsible for full payment of any services to that child.
Cancellation/No Show Policy
Patients should cancel at least 24 hours before their appointment, if possible. If a patient does not show up for an appointment without calling they may be billed for the visit anyway. If a patient no shows more than 3 times they may be discharged from our care.
If a patient knows they are going to be late, please contact the office.
We ask that patients contact our prescription refill line at least 48 hours in advance for refills.
